Character death

Player character death can be a touchy subject for some people. As most of you know I usually don't try to kill pc's, people can get pretty attached to them. A monster killing you outright with no chance of save vs death, you probably wont see unless you are bringing in a new char (if you wanted the old one to "retire" in a dramatic fashion). PC vs PC death luckily hasn't happened yet, as a rez is a possibility and it would make for an interesting quest, well.......depending on the circumstances.
PC death does add more of a sense of realism because you know there is a chance your character can die. So going into a fight with some big baddie makes the group more cautious/better planning. Some instances are unavoidable such as playing dice with an archdevil, drawing a crap ton of cards from a strange magical deck, cursed items, and playing way out of alignment/doing stupid shit. I try to limit availabilty of certain potentialy dangerous items, when the group runs across them and they decide to keep chancing fate I'm going to stop "fluffing" rolls after a certain amount of chances. Take the deck of many things for an example, if you dont have potentialy bad results, to go along with the really good ones, where is the oh fuck factor?
